BANK STANDING ORDER INSTRUCTIONS

(UK RESIDENTS ONLY)

If you have a current bank account in the United Kingdom / Northern Ireland you may wish to pay your annual membership to the Guild by Bank Standing Order, in which case do not send any money but complete the form below as instructed and post this form with the Application Form to the Guild at the address given.  A copy of your Standing Order Instructions will be sent to you with your membership pack. NOTES FOR COMPLETION OF THE BANK STANDING ORDER FORM Before completing the Bank Standing Order form look at your cheque book as you will need certain details from your cheque book for the form below.

(A) In section 1 below where asked for Your Bank Account Name fill in the name  as given on your cheque book.  For example, you may be known as John Smith but your bank account may be held in the name of “J. G. Smith and P. Smith”.  Give exactly Your Bank Account Name as given on your cheque book.
(B) Your Bank Account Number fill in Your Bank Account Number as given on your cheque book
(C) In section 1 below where asked for Your Bank Sort Code fill in Your Bank Sort Code, which is normally shown in the top right-hand corner of your cheque and is made up of six numbers, e.g. 20-36-46.
(D) Please sign and date section 4 below, after ensuring that sections 1 and 2 have been completed correctly. (Do not complete any of the details in Section 3)

Please pay to the order of The Guild of International Songwriters and Composers, account number _____________ at Lloyds TSB Bank plc, Market House, Penzance, Cornwall TR18 2TN, England, sort code ___________ , the sum of  £50.00 commencing on the               day of                             2009  and a similar amount thereafter on the             day of                                each year until I/we give notice in writing to the contrary.  This is an annual payment.
